The clinical picture of primary adrenal insufficiency (PAI) is based on the chronic deficiency of glucocorticoids and/or mineralocorticoids due to the failure of the adrenal cortex to produce these hormones in sufficient amounts. The most common cause of PAI in adults is autoimmune diseases, except in countries where tuberculosis remains endemic. Autoimmune PAI typically occurs between 30 and 50 years of age, with a peak in incidence in the fourth decade of life, and a slight female preponderance. Isolated autoimmune adrenalitis occurs in approximately 30-40% of cases of PAI. Erichsen and colleagues reported that almost two-thirds of patients with autoimmune PAI in the Norwegian registry had at least one other autoimmune disease, most commonly autoimmune thyroid disorders. It is important therefore to consider the potential for co-morbid autoimmune disorders to be present in patients with PAI, and to seek them out as a cause of unexplained morbidity.

Addison’s disease (AAD) is a kind of PAI caused by the bilateral destruction or dysfunction of the adrenal cortex, and the biochemical pattern of this condition is a deficient production of glucocorticoids, mineralocorticoids, and androgens, associated with high levels of plasma renin activity. The lack of therapeutic strategies is obvious that patients with AAD require life-long substitutive treatment with hydrocortisone. Antibody-based therapy may be a strategy but still not realize now and researchers need to study more on it.
